To facilitate the registration process, you can fill out the forms in advance so that you can save time and be better prepared for your visit to our facility. Health Insurance Plans

Irvine Orthopedics contracts with a number of different PPO and other health insurance plans to cover most cases. We have listed the insurance carriers that we are contracted with. If you do not see your particular insurance listed, you can call our office to ask if we can accept your particular health insurance plan.

Prescriptions

For all prescription refills, please have your pharmacy fax their request to our office. We will process it during regular office hours (NO WEEKEND REFILLS). Please allow 1-2 days of processing time. For narcotics/controlled substance refills, follow up is required. Please contact our office directly.
